Athletic Commitment Form

Athletic Commitment Form
  •

As a student athlete I understand my responsibility to follow the rules of Columbia Adventist Academy.  I have gone over the grade policy, the attendance policy and student behavior policy in the school handbook.  I know that my ability to participate in athletics requires me to follow these and all of the rules of Columbia Adventist Academy, and if I do not I will not be eligible to participate.

Columbia Adventist Academy Athletics is a commitment of time and energy.  If you make a team you must participate in your team’s practices.  You are not allowed to miss practices unless the Athletic Director/and or Coach has been talked to and given you permission. Going out to dinner and excuses like that are not reasons to miss.  Your coaches and teammates will be putting in lots of time and energy and need everybody there.

I understand the responsibilities of being a student athlete at Columbia Adventist Academy and I am committed to following all of the schools rules and committed to being at every practice and every game.

  • Parents, we ask that you read and sign this document signifiying that you understand the commitment that your student has made. You also agree that concerns with your athletes' playing time or other issues will be directed to the Athletic Director, not the coach(s). All issues should be addressed with the Athletic Director who will talk to the coach(s) and/or mediate if needed.

You are also agreeing to allow the coaches to coach and the officials to officiate. We ask that you do not coach from the sideline or make comments to the officials. Please leave that to the Athletic Director and coaches.

Practices are closed practices, meaning that they are for the team and coaches only.  If you feel that it is necessary to attend a practice please contact the Athletic Director so he can arrange it with a coach when appropriate.
